About Realise
Realise South West is Somerset Care’s award winning learning disabilities service.
Realise is committed to transforming care and support for people with a learning disability or Autism, focusing on empowering people and their families.
Realise South West have gained national recognition for their work on Transforming Care agenda.
The Transforming Care programme aims to improve the lives of children, young people and adults with a learning disability and/or autism who display behaviours that challenge, including those with a mental health condition. We have successfully enabled people to move out of hospital or unsuitable placements and to live more independently, growing in confidence and self-esteem. We work closely with the individual, their families as well as professionals to learn about the individual, their childhood, interests and preferences so that we support them in the best way possible.
The Realise team support people to:
- Learn and develop independence skills
- Transition from long term hospital stays into independent living
- Seek meaningful work or voluntary opportunities
- Integrate with their local communities
- Build new friendships
How can you access this support?
Those that we support may have their service funded through their Local Authority, through an Individual Budget or privately. We recommend speaking to your local authority, to see if you are entitled to this support.
We have a passionate team at Realise who care deeply about empowering people to live the life they choose, promoting positive risk taking and removing barriers.
Realise use bespoke recruitment strategies to ensure customers and their support team are well suited. Our customers are often involved in the recruitment process, empowering them to be part of their support as much or as little as they wish.
