Person-centred care for learning disabilities
At the heart of person-centred care for learning disabilities, is a determination to recognise the person, not the label. It is about providing truly tailored learning disability care that is bespoke to the individual, their needs, preferences and personal goals.
Whilst care for learning disabilities can be seen as an overarching category of support, there is no ‘one size fits all’. The support requirements of each individual will vary according to their learning disability and how this affects them on a daily basis. Person-centred care for learning disabilities involves creating a personalised support plan to enable the individual to build the life skills they need to live the life they choose.
The delivery of learning disability care aims to support people to live as independently as possible, pursue their hobbies and interests, and live a life that is meaningful to them, whilst helping them to manage their learning disability and any associated health conditions. This can include transforming care which enables an individual to move from a long-term hospital stay into their own home, provides support for the individual to transition between child and adult support services, or perhaps assist them to gain independence thanks to mobility or communication aids or a specially adapted vehicle.
Person-centred care for learning disabilities is about enablement and empowerment – helping an individual to overcome the challenges presented by their learning disability, and supporting them to build the life skills they need to lead a more independent and fulfilling daily life.
